Health Services

Medical Services. As used in this Agreement, the term “Medical Services” shall mean those medical services that the providers of WPA are permitted to perform under the laws of the State of Texas and that are within the scope of their training and licensure.

PREVENTATIVE

Members are entitled to an annual wellness WPA Provider examination and evaluation, which shall include the following as needed and determined by the WPA provider. 

  • Health Risk Assessment                               
  • EKG
  • Vision and Hearing Screening                    
  • Psychosocial Screening
  • Pulmonary Function Testing                      
  • Body Fat Analysis
  • Comprehensive Lab Screening*               
  • Custom Wellness Plan to Incl Exercise and Dietary Plan
  • *CBC, CMP, TSH, T3, T4, LIPID, PSA, UA
  • Breast and Pelvic Exams                             
  • Family Planning and Contraception
  • Depression Screening                                   
  • Nutritional Counseling
  • Pap Smears/Prostate/Rectal Exam             
  • Bone Density
  • Cancer Screening-Prostate, Cervical, Breast Exams in office

Included once a year with your annual membership, if needed more than once a year, the Office Visit fee of $25 will apply.

Some conditions may require a referral to a specialist for diagnosis or treatment.  This may be a one-time referral or an ongoing referral for management of a specific disorder not in the scope of this practice. WPA Providers do not treat conditions that require specialty treatment that is outside of WPA Providers’ scope of practice (Family Practice Medicine).  Under these circumstances, WPA providers will make a referral to a sub-specialist or other appropriate provider, and will coordinate and follow up with the sub-specialist of the Member’s choice.  The Member is responsible for any charges from non-WPA providers.

Non-Medical, Personalized Services.  The Wellness Practices of America shall also provide Patient with the following non-medical services (“Non-Medical Services”):

 

(a)   Access.   Patient shall have access to the WPA Provider that is on call outside business hours. Patient can call the main office phone and obtain the emergency number where a patient may reach the WPA Provider directly.  However all emergencies should be directed to 911 or your nearest Emergency Room.

 

(b)  No Wait or Minimal wait Appointments.   Every effort shall be made to assure that Patient is seen by the WPA Provider immediately upon arriving for a scheduled office visit or after only a minimal wait. If WPA Provider foresees an extensive wait time, Patient shall be contacted and advised of the projected wait time.  It is also important that each patient arrive on time for their appointment.  If a patient cannot make it in on time, a longer wait is possible or visit will need to be rescheduled.

 

(c)       Same Day/Next Day Appointments. When Patient calls WPA prior to noon on a normal business day (Monday through Friday) to schedule an appointment, every reasonable effort shall be made to schedule an appointment with the WPA Provider on the same day. If the patient calls or e-mails the WPA Provider after noon on a normal business day (Monday through Friday) to schedule an appointment, every reasonable effort shall be made to schedule Patient’s   appointment with the WPA Provider on the following normal business day. In any event, however, Wellness Practices of America shall make every reasonable effort to schedule an appointment for the Patient on the same day that the request is made.

 

(d) Specialist coordination and follow up.   WPA Providers do not treat conditions that require specialty treatment that is outside of WPA Providers’ scope of practice (Family Practice Medicine).  Under these circumstances, WPA providers will make a referral to a sub-specialist or other appropriate provider, and will coordinate and follow up with the sub-specialist of the Member’s choice.  The Member is responsible for any charges from non-WPA providers, but will not be charged for coordination and follow up.
